SIBIUL MEDICAL and "Carpe diem" - the right to health care in Sibiu county and the financing of family doctors' offices!|Event |Tribune

The right to health protection is guaranteed, the state being obliged to take measures to ensure hygiene and public health, in accordance with art.34, from the Constitution of Romania.The same article of the fundamental law stated in para.(3) - "The organization of medical assistance and the social insurance system for illness, accidents, maternity and recovery, the control of the exercise of medical professions and paramedical activities, as well as other measures to protect the physical and mental health of the person are established according to the law" .For many years, the public debate in the field of county health has been focused on the necessity and opportunity of building a new county hospital, with all the adjacent issues: real estate location, ensuring adequate financing, structure, equipment, ensuring specialized human resources, etc.On July 15 of this year, the Ministry of Health published a series of documents related to the financing of individual medical offices (including associations of offices), through PNRR funds. Thus, family doctors' offices can be equipped with equipment worth a total of 45,150 euros (without VAT) , but they can also be renovated/modernized, for this purpose max.15,000 euros (without VAT)/ office or associations of offices.I know - both in urban and especially in rural areas - many family doctors who would like to benefit from such facilities (without ensuring any co-financing), the costs being borne 100% from non-reimbursable European funds.I am absolutely convinced that all family doctors would like to equip their practice with the following equipment (14 positions): Point of Care analyzer, digital electrocardiograph, portable ultrasound, 3-probe ultrasound, IT equipment (desktop, laser printer), ecological recycling bins, medical waste container, vaccine refrigerator, refrigerator temperature data logger, vital functions monitor, digital spirometer (PC connection), automatic blood pressure monitoring (Holter BP 24 hours), oxygen concentrator 5 l, ankle-brachial index measurement device , but also cabinet furniture (7 positions): medical instruments, portable oral/nasal/tracheal aspirator, resuscitation kit with automatic defibrillator, room air sterilization device, fetal Doppler device and peripheral arterial flow Doppler device, gynecological examination chair, blood collected stool (3 sections).The Sibiu County House of Health Insurance publishes (online) "List of family doctors' offices that are in a contractual relationship", these being the main providers of medical services that directly serve the citizen.In total, there are 224 offices operating in Sibiu county, in urban and rural areas.It can be assumed that the situation of their endowment is differentiated, depending on a lot of factors.At the local level, several municipalities have started the construction of medico-social complexes (remember: Valea Viilor and Turnu Roșu), there are many peri-urban municipalities that want to initiate the construction of such complexes, with medical services appropriate to the needs of the local community.In the desire for a correct analysis of the situation of these offices, we have turned to the newest tool, which is under public debate: the Economic and Social Development Strategy of Sibiu County, published on the website of the CJ of Sibiu.From Annex 3 - Health diagnostic analysis, I understood that it was analyzed: the health status of the population of Sibiu county (1), the socio-economic determinants (2), the degree of illness of the population (3) and the existing health service providers at the level Sibiu county (4).The 9 types of service providers are presented here: family medicine offices, dental offices, ambulatory and specialist offices, hospitals, emergency services, rehabilitation services, home and palliative care services, medical assistance units -social and pharmacies.A special presentation has the structures under the administration of the Sibiu County Council (bed structures, staff, budgets, but also an analysis of the efficiency of the health infrastructure).A first finding for a strategy that plans the future of the county for the following years (2021-2027) is the lack of statistics for the years 2020-2021, all analyzes refer to the period 2014-2019.From these situations we learned that the distribution of family medicine offices in the territory was - in 2019 - 205 offices in the urban environment (67.7%) and 98 offices (32.3%) in the rural environment.Their needs, the interdependence relationships between these structures and the threats they face in the context of the health crisis, etc. are not presented.Even the SWOT analysis in the strategy is not more explicit, being able to note some weak points: the decrease in the birth rate in the period 2017-2020, the incidence and prevalence of most diseases is increasing (predicting an increase in the number of patients), community healthcare covers only 67, 2% of UATs, specialized ambulatory medical assistance is little represented in rural areas, the small number of dental offices (in rural areas), part of the medical equipment does not meet current requirements (physically and morally worn), etc. .The analysis does not mention the County Ambulance Service with its infrastructure and the problems encountered in the provision of pre-hospital emergency medical assistance and assisted medical transport, respectively emergency consultations at home and unassisted medical transport!The county's health needs are concentrated in nine lines (pages 211-212), their solution being reduced to the transposition of European policy objectives into objectives assumed at the national level.Thus, objective 4 - A more social and inclusive Europe - corresponds to OS10 in the field of health: Improving equal and timely access to quality, sustainable and accessible services;modernization of social protection systems, including promoting access to social protection;improving the accessibility, effectiveness and resilience of healthcare systems and long-term care services.Is it known that there is also a National Health Strategy 2022 - 2030?As a simple citizen, I ask myself the question: why do the majority of Sibiians - diagnosed with complex or severe diseases and even on the recommendation of a surgical intervention (when the symptoms persist despite the recommended treatments) - turn to doctors from recognized university centers in the medical field: Târgu - Mureș and Cluj-Napoca?Is Sibiu County prepared to ensure fair access to quality services at a cost-effective price?Analyzing the documents of the Ministry of Health (Annex 11: List of preselected family doctors GDPR), we found that out of the 3000 associations/practices of family doctors, only 32 practices were preselected to be financed and equipped with the equipment presented above.Practically, 32 offices (between 10-14% of the total number of existing offices: 303-according to the strategy, or 224-according to CNAS) will have the chance to be equipped with modern equipment.The situation raised many questions for me, which I tried to clarify with the support of the Sibiu Public Health Directorate.Here - I benefited from the operative support of the deputy executive director in public health, Dr. Adela Morar -, clarifying a first contradictory situation.The list of 3000 preselected offices (of which 2250 equipped and 750 renovated) could be identified among doctors from marginalized regions, based on the existing situations at the level of the National Health Insurance House.CNAS has such a situation, obtained through the application of Order no. 391/2015 for the approval of the criteria for the inclusion of medical practices, depending on the conditions in which the activity is carried out.Although cumbersome and little explained, the criteria are relatively clear: the dispersion of the population in the area where the activity is carried out, the small number of insured persons registered on the lists of family doctors, the availability of the necessary human resources, the availability of an adequate physical infrastructure and geographical accessibility.The Directorate of Public Health Sibiu - although it was not involved in the programming of this funding from the PNRR - distributed the lists and documents of the program in the territory, to all family doctors;now, each doctor can check if he is on the list of preselected beneficiaries (this being fully coded: locality, county, doctor's name, etc.).And yet, how many family doctors know that - from August 4, 2022 -, they can submit the funding file on the ministry's platform?All family doctors should know that there is also a list of reserve offices, which they can submit - if there are not enough requests -, within the non-competitive call: ʺfirst come, first servedʺ!There are over 180 million euros, made available to family doctors throughout the country!Who should support and drive the development of the funding files of the pre-selected GP practices, so as not to miss this unique opportunity?Who are the preselected doctors in Sibiu county?I think that the "Sibiu County Strategy" should also mention the role of the College of Physicians in Romania, as a national professional organization and institution of public interest, non-governmental, apolitical and without patrimonial purpose!Can the people of Sibiu accept - without involvement - the saying "Carpe diem" ("live the moment") and forget that two decades ago there was the magazine "Sibiul medical", with an important role in informing doctors, but also citizens?Are we ready to face the great challenges of the third decade - of the millennium (three!) - in the field of health?
